WORKERS' COMPENSATION APPLICATION

The Monroe County Medical Society reviews physician credentials and finalizes all Workers' Compensation Board applications once they are submitted to the New York State Workers' Compensation Board. This service ensures that your application will be accepted by the Workers' Compensation Board in a timely manner and that you will have no delay in providing care to injured workers. This service is provided at no cost for full time members of MCMS. Please follow the instructions carefully when submitting your application.

If you are not a full time member, there is a $500 processing fee. If you wish to become a full-time member, you may submit an application with your WC application or by clicking HERE and a portion of your fee will be applied to your membership dues.
