MCMS has just been notified by the Monroe County Department of Health of available vaccine clinics for ALL medical practice staff not working at an affiliated hospital - this included physicians, mid-levels, clinical and clerical staff.   Each staff person will need to complete this link for one of the available time slots.

They will need to bring photo ID and a work ID (if there is no work ID, then a letter on practice letterhead signed by the Practice Administrator or Manager stating that the staff person works at that practice).   They will also need to bring the complete consent form below.
